Generally speaking, NO. In Spain, it is customary to receive the insurance policy when the policy is taken out and, thereafter, simply receive the payment receipt associated with the original policy.

Yes, however all insurers have a "waiting period" until certain types of cover come into force. You should check with us as this will vary from company to company.

The approach varies from insurer to insurer, but generally speaking yes, although there are likely to be some restrictions that may exclude any treatment related to the pre-existing condition altogether or imply some kind of limitation in this regard. It is vital that you do NOT cancel any health insurance before you know what restrictions, if any, your new provider will enforce.
